Melasma and freckles Treatment : Melasma and freckles (solar lentigines) are two common skin problems caused by abnormalities in skin pigmentation (melanin), but they differ in characteristics, causes, and depth of lesions. Therefore, effective treatment requires a combination of methods.

What are Melasma and Freckles? How are they different?

Melasma

What is it: Light brown, dark brown, or grayish patches caused by overactivity of melanocytes (pigment-producing cells). It often appears in areas frequently exposed to sunlight, such as the cheekbones, forehead, bridge of the nose, and above the upper lip.
Main Causes: UV radiation from sunlight, blue light from screens, hormonal changes (e.g., taking birth control pills, pregnancy), and genetics.
Types of Melasma:
Superficial Melasma (Epidermal): Located in the epidermis, with clear borders and dark brown color. Easier to treat.
Deep Melasma (Dermal): Located in the dermis, with indistinct borders and a purplish or grayish color. More difficult to treat.
Mixed Melasma: A combination of superficial and deep melasma (most common).

Freckles (Spots)

What are they: Small brown or black spots scattered across the face or body.
Common types of freckles:
Solar Lentigines: Flat, brown spots with well-defined edges, often occurring in adults in areas frequently exposed to sunlight.
Superficial Freckles: Small, light brown spots, often found in people with fair skin or children. They enlarge or darken when exposed to sunlight and fade when sun exposure is avoided.
Hori’s Nevus: Grayish or bluish spots, often appearing on both cheekbones. They are caused by pigment cells located in a very deep layer of the skin (often due to genetics and hormones).
Seborrheic Keratosis: Raised brown or black bumps with a rough texture. These are caused by an increase in the number of epidermal cells (not just pigmentation).

2.1 Topical Treatments and Skin Care

This is the most important foundation for controlling melanin-producing cells from overacting.
Topical medications for melanin control (must be under medical supervision):
Hydroquinone: The gold standard medication that inhibits the melanin-producing enzyme (tyrosinase), but must be used short-term and under medical supervision because incorrect use can lead to “permanent melasma” (ochronosis).
Triluma / Triple Combination Cream: A combination formula of Hydroquinone + Retinoid (helps exfoliate skin cells) + Corticosteroid (reduces inflammation), highly effective in treating melasma.
Whitening skincare and extracts (readily available/highly safe):
Thiamidol: A patented ingredient recognized for its excellent and gentle melanin inhibition.
Tranexamic Acid: Helps reduce inflammation and melanin production stimulated by UV radiation.
Alpha Arbutin / Kojic Acid / Licorice Extract: Natural extracts that help fade dark spots and freckles.
Niacinamide (Vitamin B3): Inhibits the transport of melanin to the upper layers of the skin.
Vitamin C / Vitamin E: Antioxidants. Helps protect skin from sun damage and reduce dullness.

2.2 Laser and Energy-Based Devices

Suitable for those who want quick results or have deep freckles/melasma that topical medications cannot reach.
Picosecond Laser: This technology releases high-speed energy at the level of one trillionth of a second, breaking down pigment into very small particles without accumulating heat under the skin that could cause burns. It is very suitable for melasma, deep freckles, and sunspots.
Q-Switched Nd:YAG / Re-Key Laser: A traditional pigment-destroying laser. Suitable for treating superficial freckles, sunspots, and deep freckles (but caution is needed in the case of melasma, as excessive heat may darken the melasma if not set correctly).
IPL (Intense Pulsed Light): Not a laser but a high-intensity light energy. Suitable for superficial freckles and light dark spots, helping to even out skin tone.
CO2 Laser (Carbon Dioxide Laser): Used specifically for “skin tags.” It is used to cauterize the raised tissue scabs, causing them to fall off.

2.3 Clinical Treatments

Meso Melasma/Brightening Injection: Using a small needle to inject medication (such as Tranexamic acid, Glutathione, Multivitamins) directly into the middle layer of the skin to inhibit melanin production.
Chemical Peeling: Using fruit acids such as AHA, BHA, or TCA in medical concentrations to accelerate the exfoliation of the upper layer of skin containing accumulated pigment. Suitable for

2.4 Oral Medications for Melasma

Tranexamic Acid (tablets): In cases where melasma is resistant to treatment, your doctor may prescribe this medication in a low dose to help control melasma from within. However, this must be under strict medical supervision because it is contraindicated in individuals at risk of blood clots.

Self-Care and Prevention

Treating melasma will be unsuccessful if these practices are neglected:

1. Sunscreen is essential: Apply sunscreen daily, even if you don’t leave the house, as light from lamps and screens can also trigger melasma. Choose a broad-spectrum sunscreen (protecting against both UVA/UVB) with SPF 50+ and PA++++, and apply an amount equal to two fingertips.
2. Avoid heat and intense sunlight: Try to avoid sun exposure between 10:00 AM and 4:00 PM and activities that expose your face to direct heat radiation (such as sitting in front of a hot stove or using a sauna), as heat can cause blood vessels to dilate and darken melasma.
3. Avoid harsh skincare products: Using unapproved skin whitening creams or overly harsh peeling creams will thin the skin, make it more sensitive to the sun, and eventually cause the melasma to return even darker than before.

Everyday habits that unknowingly “trigger melasma”:

Heat Trigger: Melasma isn’t just afraid of sunlight, but also of “heat.” Standing in front of a stove for long periods, blow-drying with hot air directly over the skin, doing hot yoga, or even using a sauna can stimulate blood vessels under the skin to dilate and signal melanin cells to produce more melanin.
Scrubbing or wiping the face too hard: Disturbing the skin by scrubbing, rubbing, or wiping the face vigorously with cotton pads can cause chronic molecular irritation (micro-inflammation), which can stimulate dark spots and melasma to darken. Emphasis should be placed on touching the skin as gently as possible. MEDA
